• the dates of attendance
• the name of the authorised representative of your/your child’s Church/Place of Worship
• the telephone number of the authorised representative of your/your child’s Church/Place of Worship
• the email address of the authorised representative of your/your child’s Church/Place of Worship
• information you provide to support your application to the Main School and Sixth form
• the name and tutor group of a sibling who attends the School
• your Church Grouping declaration (see our Main School application forms available from: https://www.smrt.bristol.sch.uk/index.php/admissions)

We use your submitted data for the following purposes:


• to provide you with information that you request from us
• to update our records with changes to your personal information
• to process your information when considering an application to join the School
